Preparing for Your Appointment
- Download and complete the
New Patient Information Packet. - Collect any recent MRI films and other medical history you would like to discuss.
- Gather fax numbers for all physicians who should receive reports of your care.
- Write down the questions you have for the Medical Treatment Navigator.
Referrals and Authorization
- If you your healthcare insurance requires a referral for this visit, please contact your primary care physician to get a referral and authorization for your appointment.
- If you have workers compensation or an auto accident claim, bring your claim information with you. Please have your adjuster call the office (866-835-7246) or send a fax (734-995-4366) with authorization for evaluation and treatment.
